Connect services children, youth and adults with a variety of complex medical conditions.

Our team will work with healthcare and insurance providers to obtain approval for specialized products and equipment. We accept Medicare, Medicaid and most third-party insurances.

We provide assistance through a dedicated phone line for patients and healthcare professionals, as well as support from an experienced team trained to facilitate the unique challenges patients experience when ordering medical supplies.

We also help with determining if the patient would benefit from additional support from a dedicated product coordinator based on the complexity of the condition.

Self-Care Tips for Caregivers

Caregiver is just the right name for you – because you care.

Your loved on relies on you, and it’s a responsibility you take seriously.

But with so much time and attention focused on someone else’s needs, it can be easy to forget that you need to tend to your own.

Check in with yourself once a month to make sure you’re following all of these self-care tips for caregivers.

Rosetta Ishman

Rosetta Ishman

Product Coordinator for the Continuum Connect Team

Rosetta Ishman came to CHC with a 25-year background in medical equipment. While she has experience in customer service and office management, her heart has always been with the patients.

“Continuum Connect is very unique. I never worked in a program that has such a broad scope of diagnoses – from autism to MS to cystic fibrosis. It’s a lot of specialized equipment, and everything from incontinence products to feeding tubes. So, I get to know my patients on a very personal basis. And that’s why I like this position so much.”

Rosetta also works with insurance companies to make sure that patients aren’t surprised by any unexpected out-of-pocket costs.

“If I don’t treat the patient as if they’re my own grandmother or my own child, or someone else in my family, then I’m not doing them justice. I don’t know another company out there that is going to be able to build relationships like CHC, especially with Continuum Connect. And at the end of the day, you know that we’re going to help them get what they need to provide a better quality of life.”
